How to Enroll
ADMISSION REQUIREMENTS (Please send through email)
- Report Card with Learner’s Reference Number
– previous school year signed by the School Principal
– current school year
Student applicants must have an Academic grade of 80% in each subject and a Conduct grade of
85% or its equivalent. - Original Birth Certificate issued by PSA
- Original Baptismal Certificate
- Medical Certificate
- Latest 1×1 colored picture
- Certificate of Attendance (for Kinder applicants only)
- Application Form
- Recommendation Forms
– Academic Recommendation Form to be accomplished by the Principal/School Head
– Confidential Recommendation Form to be accomplished by the Adviser or Guidance Counselor - Medical Records
NOTE: If the requirements will be submitted onsite, the original copies of the Report Cards, Birth Certificate and Baptismal Certificate are required. Please bring two (2) photocopies of each of the documents.
Enrollment (ONLINE)

2.) Subject Email: Last Name-Level Applying (For Grade 11, please identify the academic strand.)

3.) Attach the following documents in PDF Format
*** Pre-Kinder: Checklist or Report Card
*** Kinder to Grade 11: Report Card with LRN
Documents submitted will be evaluated. The applicant‘s parents will receive an email/call after the evaluation of the requirements submitted.

Wait to be notified for a schedule of an online interview by the Guidance Associate. Results will be released by the Office of the Guidance Wellness Center.
Enrollment (ONSITE)
Present photocopies of the admission requirements, including the 2 copies of the 1×1 pictures for evaluation at the Office of the Registrar.
Pay application fee at the Accounting Office.
