How to Enroll

STEP 1: Registrar’s Office.

The registrar’s office will advise you the Subjects in the grade level through the admin portal.

STEP 2: Accounting Office.

• Pay your Enrollment Fees and get your official receipt.
• Your permanent student ID number is written on the official receipt.

STEP 1:  Guidance Office

• Submit your requirements:

1. 1. Report Card (original and photocopy)
   2. PSA Birth Certificate (original and 2 photocopies)
   3. Baptismal Certificate (original and 2 photocopies)
   4. Certificate of Good Moral Character
   5. Medical Certificate
   6. 2pcs 1×1 picture

Early Childhood Care and Development (EECD) Checklist

For foreign students:

Alien Certificate of Registration (ACR) and Copy of Passport

• Take the Entrance Exam.
• Get a Permit to Enroll.

STEP 2: Register online or onsite

• Create your CHST student portal account (at home for your convenience or in the allotted computer area in the school) at
  https://chs-tarlac.schoolautomate.com/

STEP 3: Go to the Registrar’s Office

• Bring your permit to enroll and Report Card.
• Then, the registrar’s office will advise you the Subjects in the grade level.

STEP 4: Go to the Accounting Office.

• Pay your Enrollment Fees and get your official receipt.
• Your permanent student ID number is written on the official receipt.
